Homeowner Affidavit: I hereby certify that the work described on this permit application shall be installed by myself in my home in which I live or intend to occupy. All work shall be in accordance with State Codes and Local Ordinance and shall not be enclosed, covered or put into service until approved by the Code Official.

Applicant Signature Notice (PA 230, Sec. 23a): Section 23a of the State Construction Code Act 230 of 1972 prohibits a person from conspiring to circumvent the licensing requirements of this State relating to persons who are to perform work identified in the Skilled Trades Act 407 of 2016 as requiring licensure.

General: Work shall not be started until the application for permit has been filed. All installations shall be in accordance with applicable codes. Inspections shall be scheduled electronically or by phone. It is the permit holder’s responsibility to provide access and the necessary tools for inspections or testing. Failure to do so may result in additional fees.

Expiration of a Permit: Permits expire and are closed 180 days from the issue date (if there is no activity) or from the last inspection. A single extension may be granted by the Code Official if requested before expiration of the permit. Closed permits may be reactivated for half of the original fees within 365 days of the original issue date.

Inspections: A single commercial inspection consists of drive time and up to 15 minutes on site. Each subsequent ½ hour on site (or portion thereof) may be considered an additional inspection subject to additional inspection fees at the discretion of the Code Official.

Fees: Permits are not valid unless all fees are paid.
